Case Questionnaire — Patient Details

Please start with a few details about the patient. These details decide which questions follow, so the questionnaire fits the patient exactly. It then takes about 30–45 minutes, and your progress is saved automatically as you go.

Part 1 is the questionnaire in front of you.

Part 2 arrives by email a few minutes after you submit. Your homeopath reviews your answers and, in most cases, sends back a short list of follow-up questions — the specific details needed to choose the right remedy that the questionnaire could not ask in advance.

The email is titled “Important: Please provide more information for your upcoming appointment” and contains a button that opens the questions. You can type or speak your answers.

Please answer that email before your appointment. Cases where the follow-up questions go unanswered are the most common reason an analysis is delayed or a remedy has to be chosen on incomplete information.

Part 2 arrives by email a few minutes after you submit this questionnaire. You will receive an email either way — so if nothing has arrived within an hour, something has gone wrong and you should tell us.

That email will say one of two things:

  • More questions, if your homeopath needs them. This is normal and happens with most first submissions — it means specific details are needed to choose the right remedy that this questionnaire could not know to ask in advance. The email is titled “Important: Please provide more information for your upcoming appointment” and has a button that opens the questions; you can type or speak your answers. When you send those answers you get a second email confirming they arrived and your case is complete.
  • An acknowledgement that nothing further is needed — you filled this in fully enough, and your case is already complete.

Either way, once you have that confirmation you are ready to go ahead with your appointment with the doctor, online or at the clinic.

If the email asks for more information, please answer it before your appointment. Cases where the follow-up questions go unanswered are the most common reason an analysis is delayed or a remedy has to be chosen on incomplete information.

  • Give us an email address you actually check.
  • If it has not arrived within an hour, check your Spam, Junk and Promotions folders — and mark it “not spam” so the rest of your clinic mail reaches you.

The patient

Just type the digits — day, month, year. For example, type 22071971 for 22 July 1971; the slashes are added for you. If completing this for a child, enter the child's date of birth.

This decides which body-system and reproductive questions appear.

Your follow-up questions are sent here after you submit. Please use an address you check — without it your homeopath has no way to ask you the remaining questions.

This must be their address, not yours. After you submit, we email them a short set of questions to answer themselves — in their own words, privately. It takes about five minutes and it makes a real difference to the remedy choice.
